To make your visits go as smoothly as possible, please read the information below

  • Arrive 15 minutes early to your first office visit to review and complete any necessary paperwork.

  • Complete the forms in advance to save time at your appointment. (Please see the forms below.)

  • Bring your insurance card and a valid photo ID to each appointment to ensure your account is billed correctly.

  • We offer early morning and late afternoon appointments at our location.

Prepare for Your First Appointment.

Whether it’s your first time attending physical therapy or you’re coming back for more, we are here to answer all your questions. Our expert staff and team of experienced therapists want you to feel at ease when you arrive for your first appointment. Here are some of the common questions we hear from patients.

How do I schedule an appointment?

To schedule an appointment at our Oasis Rehabilitation Services, simply visit our website and navigate to the Schedule Section or Free Consultation link. Fill out the request form, and our team will get back to you to confirm your appointment. If you prefer, you can also call our clinic directly at 305-606-4836 or email us at Therapy@oasisrehabservices.com for assistance. We’re here to make the process as easy and convenient as possible!

What should I expect at my first appointment?

For your first appointment, please plan to arrive 10-15 minutes before your scheduled time. We are a healthcare office, so there will be some paperwork and forms to sign. Parents/guardians: For patients under the age of 18, please come with your child or make arrangements to sign paperwork before your child’s first appointment. Things to bring: Insurance card(s), state ID, a list of medications you are currently taking and if your doctor ordered therapy, please bring in your referral or prescription.

What should I wear?

We want you to be comfortable during your visits. You are welcome to come in your street clothes or change into something comfortable when you get here. If we are evaluating your knee or shoulder for the first time, you may choose to bring a T-shirt or shorts, but it is not required. We do suggest comfortable shoes with non-skid bottoms for your comfort and safety. However, some of our pediatric locations do not allow shoes.

How much does PT cost?

At Oasis Rehabilitation Services, we provide direct access to physical therapy for patients in need. While we make the process as smooth as possible, it’s always a good idea to reach out to your insurance company before starting treatment. You can inquire about the details of your policy’s coverage for physical therapy, as well as whether the provider is within your network. What types of forms do I need to fill out prior to my appointment?

Like most healthcare visits, you will need to fill out appropriate forms when you arrive for your first visit.

How long do I need to attend physical therapy?

After your initial evaluation, your therapist will recommend a personalized treatment plan, including an estimated length of care. The recommended number of treatments can range from several visits to several months depending on the severity of your injury and the level of function you seek to attain.
